Avita Community Partners has an exciting opportunity to make a difference in people's lives on a daily basis
Successful applicant will work with HUD Grant and Georgia housing grant clients in a supported living environment. Will assist in the expansion and further development of the supported housing program. Applicant may also work directly with supported housing clients.
Manage the Georgia Housing Voucher Program-
Process referrals, interview clients, communicate with other Avita Staff, develop relationships with housing providers in the community, assist with locating housing, meeting with landlords and inspect housing for Housing Quality Standards, lease signings, request checks be cut from accounting to pay deposits, utilities rent, submit reimbursement request to DBHDD to get Avita reimbursed monthly, complete and submit required monthly reports to DBHDD, coordinate with Georgia Department of Community Affairs for Section 8(Housing Choice Voucher), assist client complete application, submit application, request meeting for client to transition to Housing Choice and request inspection by DCA, of unit for transition. Follow up with landlords and clients to renew leases, meet with clients and/or landlords if issues arise either client or lease (payment issues etc.…). Link clients to other community providers for needed resources (SNAPS, food banks, furniture banks, clothing closets etc...) Assist clients with gaining income; refer to SOAR specialist, supported employment etc.…
Provide direct service to clients and document services provided
Provide needed residential services to clients in the programs, documentation of all services provided.
Learn other residential processes
Learn Shelter Plus Care Referral Process, Supported Apartment Referral process. Crisis unit referral, reimbursement, and reporting process, Forsyth Transitional Housing referral process and operations. Learn the workings of HUD grants. Become trained and proficient in the HMIS Client Track system and required documentation in this data base as DBHDD partners with DCA it will become necessary to enter clients into Client Track). Rent Collection, Deposits, Voucher Request, Purchase Orders etc.… Communicating with Leasing Office/landlords regarding repairs needed on units. Scheduling contractors as needed for moving a client, cleaning a unit, carpet cleaners, large repair bids etc.
High school diploma or GED and three years experience in a social services related position. Or Bachelor's degree in a social services related field from an accredited college, and one year experience in a social services related position Or One year at the lower level (SST2).
Must have valid drivers license and no driver may have more than two chargeable accidents, moving violations or DUI's in a three year period with the seven year MVR report.
